do release and forever discharge Simply A Maizing, Sporty Acres, LLC,  and Maple Lawn Entertainment, Inc., including; William and Sigrid Smith, Brian and Jennifer Howard and Rodny and Angela Smith,  their heirs and personal representatives, and all firms, corporations, and persons on their behalf liable, from any and all claims, demands, damages, actions, or causes of action arising from or growing out of any and all personal injuries and/or property damage that my child, _________________________, may incur while patronizing Simply A Maizing, including the mazes, hayride, pumpkin patch, Geo Caching, and any other games, activities, entertainment or events.

I HAVE READ THE ABOVE AND FULLY UNDERSTAND ITS PROVISIONS. I ALSO ACKNOWLEDGE THAT I HAVE READ THE FARM RULES AND AGREE TO ABIDE BY SAME AS WELL AS ALL OTHER RULES POSTED ON THE FARM.
